Apply for Services
Intake process
You are one step closer to an inclusive community.
Before you apply for services, please ensure you are eligible for funding through the Government of Alberta Persons with Developmental Disabilities (PDD) program.
What to expect
Assessment & Support Determination
When you are funded through the Persons with Developmental Disabilities (PDD) program, your PDD worker will meet with you to help plan the services that meet your needs and goals.
After the meeting, your PDD worker creates an Outcome Plan. You will share it with the service providers you choose (like Chrysalis) so they can help you develop your Individual Support Plan.
Once you have your Outcome Plan and list of providers you want to meet, you are responsible for booking intake meetings.
Service Recommendation Discussions
When you come to Chrysalis for your initial assessment meeting, our Intake Specialist will discuss service options at Chrysalis with you and your support network. This conversation helps us understand your interest in pursuing services with Chrysalis.
If you choose to move forward, Chrysalis will submit an Individual Service Request and budget to PDD for approval.
Funding Approval and Staff Selection
Once we receive written approval for funding from PDD, we begin selecting and hiring staff if necessary. After staffing arrangements are finalized, we will identify a start date for your services.
We are committed to making this process as smooth and transparent as possible, ensuring you receive the support you need. If you have questions, please contact us at intake@chrysalis.ca.

Questions?
At Chrysalis, we understand how important it is to receive the support you need. Our team carefully reviews every request for services. Our review process is thorough and includes verifying documents and information gathered during your interview and from PDD assessments. This ensures that we have a complete understanding of your situation.
If we determine that you are not eligible for our services, please know that this decision is never easy for us. We will inform you of the reasons behind the decision and guide you on consulting with PDD for alternative support options that may be available.
If you feel that the decision does not reflect your situation, you have the right to appeal.
If you wish to appeal the decision, you can submit a written or verbal notice to our Intake Coordinator within two weeks of receiving the ineligibility notice. The appeal will be reviewed promptly, and we will provide a decision within one week. We will communicate the results clearly, ensuring you understand the outcome and any next steps.
At Chrysalis, we are committed to ensuring you receive the support you need, whether through our services or by helping you find alternative options. We are here to help, every step of the way.
